Sacramental record requests must be made through the parish at which the Sacrament of Baptism was administered.
Sacramental records are private, confidential documents. Certificates are only issued to the person whose record is requested or to a person who provides proof of parental authority, guardianship, power of attorney, executor/personal representative, or other legal status on behalf of the person whose record is requested.
For individuals under 18 years of age, a parent or legal guardian may request the form on their behalf.
Individuals 18 years or older must submit the request themselves.
No official certificates are issued for genealogical purposes.
We provide newly issued baptismal certificates with notifications for Sacramental preparation, Marriage Preparation and Annulments directly to the requesting parish. Please provide the preparing parish’s mailing address and indicate the appropriate recipient for the envelope. You will be notified when the certificate has been sent.
